As a parent/legal guardian of the student named above, I understand and agree to the following:
- This form is valid for the duration of a student's participation in the dual credit/early admission program.
- In order to enroll and receive college credit for dual credit/early admission courses, a student must meet guidelines of the high school and the college and be approved to take dual credit/early admission courses.
- A student must follow all policies of Howard College and the school district.
- The social security number provided may be used to update Howard College records, if not previously provided.
- All forms and documentation, including the Howard College application for admission, student/parent agreement, and test scores (TSI, ACT, or SAT), must be submitted before enrollment/registration.
- Requirements of the Texas Success Initiative (TSI) must be met for the subject area(s) required.
- A student must remain enrolled in high school.
- A student is not officially enrolled until all required registration documents and payment have been submitted to the college. Payment is due at the time of enrollment. If payment is not made at that time, a student may be dropped for non-payment.
- Dual credit/early admission courses are college-level. College-level courses may include controversial, sensitive, and/or adult material. Students are expected to have readiness for college-level rigor and content.
- A student must notify Howard College by completing the withdrawal process by the last date to withdraw (see college calendar) if they decide to drop or withdraw from courses.
- A student may be enrolled in interactive videoconference classes that are transmitted from the college to the high school if scheduled by the high school. These classes may be recorded and uploaded to a password-protected website link.
- Proctoring may be required for students enrolled in online courses. Proctors at high school campuses may have access to student grades, due dates, and course announcements.
- Grades earned in dual credit courses become part of a student's permanent academic record and will be reflected on the college transcript.
- A student will not receive an official Howard College transcript until an official transcript from the high school is submitted to the college that includes a graduation date.
- A student's signature in the online application gives the high school permission to send Howard College all required test scores, enrollment documentation, and a final transcript upon graduation.
- A billing statement is available on the student's online portal.
- Per federal regulations, students (including dual credit) not meeting satisfactory academic progress standards may be placed on financial aid warning, even though they are not receiving financial aid. Students who do not meet satisfactory academic progress requirements during the next semester of attendance following their financial aid warning will be placed on financial aid suspension. Please refer to the current college catalog for the most current information.
- Directory Information is listed as follows: Student's full name, address, telephone number, date and place of birth, major field of study, participation in recognized activities and sports, photograph, weight and height for athletic teams, dates of attendance, degree(s) and awards received, most recent previous school/college/university attended, and classification. The student has indicated on the online application whether they want their directory information released or withheld. This will affect whether the student's name appears in event programs, such as college graduation programs, and whether their name is published on the college website for any recognition received.
- Under federal legislation, namely the Family Educational Rights and Privacy Act of 1974 (FERPA), educational records cannot be released without written consent. The student has indicated on the online application whether educational college records (grades and transcripts, financial records, including financial aid, disciplinary records, and honors) and educational information may be released to parent(s)/legal guardian(s).
I, as a parent/guardian of the above-mentioned student, state that I have read and agree to all conditions and requirements of enrollment and registration outlined above. I give permission for the above-mentioned student to enroll in Howard College's dual credit/early admission program.
