4. Go to the Radiologic Technology webpage and review requirements specific to the radiologic technology program.

   From this point on, you will fill out all the required information for CastleBranch to complete your background check.
   
   
6. Use the link above through CastleBranch, in the dropdown choose (San Angelo), (Radiologic Technology), HZ78fall: Fall 2026 Compliance Tracker. Order the Immunization Compliance Tracker to upload your documentation of the following immunizations: Hep B, MMR, Varicella, Tdap, and PPD (TB clearance). **This will be numbers 1-5 on the tracker.**
   
   
7. If you applied in 2025 and ordered an immunization Compliance Tracker but were not accepted into the program then you may use last year's tracker; please just make sure all information is currently up to date.

The following items will be needed to upload and attach to your online application:

1. Your TEAS test results (upload the score sheet that shows separate scores in each category of Reading, Math, Science, and English)
   
   
2. A copy of all your unofficial transcripts, including Howard College Transcripts, to show letter grades earned for prerequisites from all colleges attended, including any dual credit classes.

If I am accepted as a student at Howard College, I will honor all administrative summons and conform to and abide by the letter and spirit of all the rules, regulations, and procedures of Howard College as given in the college catalog and student handbook. I hereby give the right to disclose to appropriate college officials my school records for use in planning my program. I certify the above information is complete and correct.
